- Fill the Cashless Pre-authorization Form available at Hospital Counter.
- The Hospital Cashless Counter will fax the Pre-authorization Form and documents to the L&T Insurance.
- The Claim Reference Number will be sent to the Insured, and approval/rejection of Claim will be intimated to the Policy Holder within 6 hours.
- On Acceptance of Claim by L & T Insurance, the Bills are directly settled with the Hospital by the Insurer when the Insured is discharged from the Hospital.
- If the L&T Insurance Claim is rejected by the Insurer, apply for Reimbursement.
L and T Insurance Claim Process for Cashless Health Insurance of Planned Hospitalization
- The Insured have to select the Network Hospital from the List of L&T Network Hospital List for treatment.
- The Insured have to inform the L&T Insurance about the Admission in Network Hospital at least 72 hours before Admission.
- Fill the Pre-authorization Form available at Hospital Counter.
- Fax the Pre-authorization Form along with the required documents to the L&T Insurance.
- Claim Reference Number will be sent to the Insured, and approval/rejection of Claim will be intimated to the Policy Holder within 6 hours.
- If the Claim is Approved, the Insured Hospital Bills will be directly settled to the Hospital by the Insurer.
- The Insured have to pay the Hospital Bills and apply for Reimbursement if Cashless Claim is not approved.
